Notes

1. Entrants must have a handicap of 16 or less when application is made.   Proof of current handicap must accompany this Entry Form.  16 is the maximum handicap that will be allowed on the day.
2. No entry will be accepted without the appropriate entry fee.   If required, receipt can be acknowledged by email.     Acknowledgement required YES/NO      

3. A draw will be carried out in May and no entry money will be refunded for withdrawals once the draw is made.  In the event of the competition being abandoned due to weather, or for any other reason, the event will not be re-scheduled during 2008.   In such circumstances, entry money will not be refunded.
4. The Lowe Memorial Trophy is awarded to the team with the three best gross scores for the day from a nominated team of four.   All four players must be from the same club.    If you are a member of a team, please include a note of the names of the other members of your team.
